Please accept our sympathy & prayer at this time of your loss. We have prepared these questions to assist in selecting the options available in planning the funeral. We are also happy to plan the celebration for you if that would be of greater assistance.
Please take a moment to consider these before your meeting with our parish staff.
1. Some Initial Information
The Birth Name of the Deceased: __________________________________________________
Age: __________________________________________________
Their Common –or– Nickname: __________________________________________________
2. Please consider which form of Funeral Service you wish to celebrate.

This form of the funeral service may be most appropriate for the Catholic family who, for whatever reason, has not been active in the faith or in attending Mass. Consideration must however be given
to the wishes of the deceased.
** When the Funeral Service is chosen, the priest or deacon will complete the rest of the
funeral planning appropriate to a funeral home service.
[ ] We wish to celebrate a Funeral Mass with Eucharist.
This is the most traditional form of funeral service and appropriate for the Catholic family who is
active in their faith and the attendance of Mass. Once again, consideration should be given to the
wishes of the deceased.
3. Selecting the Readings from Sacred Scripture
Three passages of Sacred Scripture are always part of the Rite of Christian Burial: the Old and New Testaments, and the Gospels. Please take a moment to read the passages provided as part of this website to make your selections.
Consideration needs to be given to who will proclaim the Scripture. It is strongly recommended this be done by an individual who is trained in this ministry. Because funerals are a difficult time for public speaking, our parish is ready to provide trained ministers of the Word. If you strongly wish family or friends to proclaim the readings, please indicate below.
Old Testament is:______ (ie: C4) [ ] read by a Minister.
[ ] We are asking_____________________________________________ to read.
New Testament is:______ (ie: E5) [ ] read by a Minister.
[ ] We are asking_____________________________________________ to read.
The Gospel is: ______ (ie: G1) This passage is always read by the priest or deacon.

2. During a Funeral Mass, the Gifts of Bread and Wine may be presented.
[ ] We do not wish to present the gifts.
[ ] They will be presented by the following: 1.)__________________________2.)_____________________________
5. Words of Remembrance
The Rite of Funerals provides for one (1) individual to speaks on behalf of the family, for approximately 3-5 minutes in length. These “Words of Remembrance” should focus especially on the person’s living of the faith.
[ ] We will not be sharing the optional “Words of Remembrance”.
[ ] We will have “Words of Remembrance”, given by:________________________________________________ .
6. And Finally Music when celebrating a Funeral Mass
Music is an important part of our celebration of faith. You may have personal requests regarding the music and songs to be used. Please remember that music for a Funeral must be religious in nature (secular/popular music is not used for the Funeral Masses). We are more than happy to help provided suggestions to assist you. Please indicate your music requests below, and we will do our best to accomodate them whenever appropriate or possible:
